Loan forgiveness for a long-time nonprofit/public interest employee

I have a student loan that I have been paying forever.  I have been paying it since 1996 and finally have it down to 4,000+ . 

I have been working with nonprofits most of my career life – I’m 52.  This year has been extremely difficult financially for the nonprofit I work for .  This, needless to say, has me looking at restructuring who and what I owe.  I was excited to learn about the Public Service Loan Forgiveness and am hopeful I might qualify for something like this - I would love to have the remainder of my loan forgiven if at all possible. 

Can you give me information on this?  How do I apply?  Which Public Service programs qualify?
