How does consolidating my student loans effect the 25 year forgiveness period?

Does the 25 year period start over after consolidating or does the government keep track of the individual loan amounts that were consolidated?

I believe the ICR repayment option started in 1994 so that means in another 4-5 years some people may be eligible to do this, correct?  Are there any forms available to do this yet?
